ABSTRACT

Objective: To examine the outcome of different incision selection for nipple-sparing mastectomy (NSM) with implant-based breast reconstruction (IBBR). Methods: Ninety-Two cases accepted NSM and IBBR from totally 1 002 cases of breast cancer admitted to Beijing Tongren Hospital Affiliated to Capital Medical University from June 2013 to December 2020 were analyzed retrospectively. All the patients were female, aging (42.0±7.5) years (range: 27 to 64 years). There were 12 patients at stage 0, 47 patients at stage Ⅰ, 17 patients at stage Ⅱ, and 16 patients at stage Ⅲ. The patients were divided in three groups by the incision: peri-areolar group (n=23), inframammary group (n=33) and lateral group (n=36). The age, tumor size, lymph node status, hormone receptor, operation method and postoperative complications were followed-up of three groups and compared by one-way ANOVA, χ2 test or Fisher's exact test and expansion to comparied. Results: There was no significant difference in operation time ((180.7±54.0) minutes vs. (176.9±48.1) minutes vs. (194.6±37.5) minutes, F=1.401, P=0.252), postoperative drainage volume ((497.5±226.0) ml vs. (495.4±182.5) ml vs. (519.8±172.0) ml, F=0.167, P=0.846) and drainage time ((8.8±3.8) days vs. (8.0±2.5) days vs. (8.3±2.9) days, F=0.542, P=0.583) among the peri-areolar, inframammary, and lateral groups. The cumulative postoperative complications were relatively higher in the peri-areola group (26.1%(6/23) vs. 0 vs.8.3%(3/36), χ²=9.675, P=0.004). All female patients were followed up for (49.0±25.6) months(range: 12 to 112 months). Breast cancer related death rate was 2.2% (2/92), local recurrence was 1.1% (1/92), regional recurrence was 4.3% (4/92), distant metastasis was 6.5% (6/92), and the disease-free survival rate was 88.0% (81/92). Conclusion: The surgical effects of the peri-areolar, inframammary and lateral incisions for NSM and IBBR are approximate, the total complication rate of the peri-areolar incision is slightly higher.

ABSTRACT

Objective: To explore the surgical strategy of nipple areola complex (NAC) management in central breast cancer. Methods: A retrospective analysis was conducted on 164 cases of central breast cancer who underwent surgery treatment from December 2017 to December 2020 in the Breast Center of Beijing Tongren Hospital, Capital Medical University. Prior to the surgery, the tumor-nipple distance (TND) and the maximum diameter of the tumor were measured by magnetic resonance imaging (MRI). The presence of nipple invagination, nipple discharge, and nipple ulceration (including nipple Paget's disease) were recorded accordingly. NAC was preserved in patients with TND≥0.5 cm, no signs of NAC invasion (nipple invagination, nipple ulceration) and negative intraoperative frozen pathological margin. All patients with signs of NAC involvement, TND<0.5 cm or positive NAC basal resection margin confirmed by intraoperative frozen pathology underwent NAC removal. χ(2) test or Fisher exact test was used to analyze the influencing factors. Results: Of the 164 cases of central breast cancer, 73 cases underwent breast-conserving surgery, 43 cases underwent nipple-areola complex sparing mastectomy (NSM), 34 cases underwent total mastectomy, and the remaining 14 cases underwent skin sparing mastectomy (SSM). Among the 58 cases of NAC resection (including 34 cases of total mastectomy, 14 cases of SSM, and 10 cases of breast-conserving surgery), 25 cases were confirmed tumor involving NAC (total mastectomy in 12 cases, SSM in 9 cases, and breast-conserving surgery in 4 cases). The related factors of NAC involvement included TND (P=0.040) and nipple invagination (P=0.031). There were no correlations between tumor size (P=0.519), lymph node metastasis (P=0.847), bloody nipple discharge (P=0.742) and NAC involvement. During the follow-up period of 12 to 48 months, there was 1 case of local recurrence and 3 cases of distant metastasis. Conclusions: For central breast cancer, data suggest that patients with TND≥0.5cm, no signs of NAC invasion (nipple invagination, nipple ulceration) and negative NAC margin in intraoperative frozen pathology should be treated with NAC preservation surgery, whereas for those with TND<0.5 cm or accompanied by signs of NAC invasion, NAC should be removed. In addition, nipple reconstruction can be selected to further improve the postoperative appearance of patients with central breast cancer.

BACKGROUND@#Mastectomy techniques have been extended to nipple-sparing mastectomy (NSM). This study aimed to assess the actual application of NSM in China and identify the factors influencing postoperative complications.@*METHODS@#The clinical data of 615 patients (641 surgeries) undergoing NSM from January 1st, 2018 to December 31st, 2018 at 28 centers nationwide were retrospectively analyzed to obtain the rate of NSM and investigate factors related to NSM surgery.@*RESULTS@#The proportion of NSM surgery performed in this study was 2.67% (17/641). Malignant breast tumors accounted for the majority of NSM surgery (559/641, 87.2%). A total of 475 (77.3%) patients underwent NSM combined with reconstructive surgery. The rate of reconstruction decreased with age in our study, and implants were the most common option (344/641, 53.7%) in reconstruction. Radial incision was the most selected method regardless of reconstruction. However, for those who underwent reconstruction surgery, 18.4% (85/462) of cases also chose curvilinear incision, while in the simple NSM surgery group, more patients chose circumareolar incision (26/136, 19.1%). The tumor-to-nipple distance (TND) influenced postoperative complications (P = 0.004). There were no relationships between postoperative complications and tumor size, tumor location, histologic grade, molecular subtype, nipple discharge, and axillary lymph nodes.@*CONCLUSIONS@#NSM surgery is feasible and only TND influenced postoperative complications of NSM surgery. But the proportion of NSM surgery performed is still low in nationwide centers of China. The selection criteria for appropriate surgical methods are important for NSM in clinical practice. Resumen Objetivo: Se presenta experiencia realizada en Servicio de Salud Talcahuano, de masculinización mamaria en el proceso de reasignación sexual, de transexuales masculinos. En Chile se estima que habrían 754 mujeres transexuales. Materiales y Método: Estudio de 47 individuos, intervenidos en Hospital Las Higueras de Talcahuano, entre 2006 y 2019, sometidos a masculinización mamaria mediante mastectomía subcutánea y reconstrucción del complejo areolomamilar. Resultados: Se intervinieron pacientes de todo el país; 59% corresponde a octava región; 42,5% operados entre los años 2017 y 2018. Un 36% rango de edad de 18 a 22 años. Un 68% IMC entre 24 y 30. Según el tamaño de las mamas, 57% copa B, 31% C, 12% D y 12% de gigantomastía. Un 29%, peso promedio de la mastectomía fue de 600 g; un 29% de 300 gramos y en 9% más de 800 g por lado. En 68% se utilizó abordaje submamario e injerto de piel total del complejo aréola pezón (CAP); otras tecnicas fueron periareolar, incisión en T y liposucción. La complicación más frecuente fue la colección. Biopsia normal en un 100%. Discusión: Experiencia pionera en Chile, realizada en un Hospital Público, en el difícil proceso de reasignación de género, en la población transexual de Chile. Individuos muy estigmatizados de parte de la comunidad, que encontraron en el Servicio de Salud de Talcahuano, una mano amiga, que les extendió su apoyo desinteresado y calificado, logrando excelentes resultados finales.


Aim: Describe the experience in Talcahuano Helath Service, of mammary masculinization in the sexual reassignment process, of male transsexuals. In Chile it is estimated that there will be 754 transsexual women. Materials and Method: Study of 47 individuals, operated at Las Higueras Hospital in Talcahuano, between 2006 and 2019, subjected to breast masculinization by subcutaneous mastectomy and reconstruction of the areolomamilar complex. Results: Patients from all over the country were operated; 59% corresponds to the Bio-Bio province; 42.5% operated between 2017 and 2018. A 36% range of ages from 18 to 22 years. 68% BMI between 24 and 30. According to the size of the breasts, 57% B cup, 31% C, 12% D and 12% gigantomasty. At 29%, the average weight of the mastectomy was 600 grams; 29% of 300 grams and 9% more than 800 grams per side. In 68%, the submammary approach and total skin graft of the CAP were used; Other techniques were periareolar, incision in T and liposuction. The most frequent complication was the collection. 100% normal biopsy. Discussion: Pioneering experience in Chile, carried out in a public hospital, in the difficult process of gender reassignment, in the transsexual population of Chile. Individuals very stigmatized by the community, who found in the Health Service of Talcahuano, a helping hand, which extended their selfless and qualified support, achieving excellent final results.

ABSTRACT

Introdução: Sabe-se que o câncer de mama é hoje a segunda neoplasia mais frequente entre as mulheres, as quais são submetidas desde a tratamentos clínicos até cirúrgicos mutilantes da mama. Ter mamas bonitas e harmoniosas é um direito legítimo, sendo assim o conceito de reconstrução mamária e do complexo areolo papilar se torna importante na reinserção social feminina. Este estudo visa apresentar uma técnica eficaz de reconstrução areolar, utilizando enxerto cutâneo livre palpebral bilateral, demonstrando sua eficácia através de resultados fotográficos, comparação com outras técnicas já descritas e questionários de satisfação dos pacientes. Resultados: Foram avaliados, retrospectivamente, 50 pacientes operados de reconstrução areolar com enxerto de pele palpebral e outras técnicas já descritas, operados por um único cirurgião sênior. O grupo de pacientes operados com pele palpebral não obtiveram diferenças significativas dentro da análise de resultados, comparados com outras técnicas de reconstrução. Houve boa satisfação por parte dos pacientes, quanto ao resultado das cirurgias das aréolas. Conclusão: O estudo obteve baixos índices de complicações, apresentando-se como boa opção terapêutica, dentro do arsenal cirúrgico de reconstrução areolar.


Introduction: Breast cancer is currently the second most common neoplasm among women who undergo clinical treatments and breast-mutilating surgeries. Having beautiful and harmonious breasts is a legitimate right, so the concept of reconstructing the breast and nipple-areolar complex becomes important in female social reinsertion. This study aimed to present an effective technique of areolar reconstruction using free bilateral eyelid skin grafts and demonstrate its effectiveness through photographic results, comparison with previously described techniques, and patient satisfaction questionnaires. Results: The authors retrospectively evaluated 50 patients who underwent areolar reconstruction with eyelid skin grafts and other techniques performed by a single senior surgeon. Patients who underwent eyelid skin graft surgery did not achieve significantly different results from those who underwent other reconstruction techniques. Patients who underwent areola surgeries reported good satisfaction. Conclusion: The study revealed low complication rates and demonstrated that the use of eyelid skin is a good therapeutic option for areolar reconstruction.

Introdução: A reconstrução do complexo areolopapilar (CAP) é etapa fundamental no tratamento do câncer de mama. Tipicamente, é realizada em dois tempos distintos, sendo primeiro a papila reconstruída e somente após sua completa cicatrização que a tatuagem é realizada. Métodos: Estudo retrospectivo realizado no período de 2015 a 2016 no Hospital São Lucas da PUCRS. Foram incluídas 21 pacientes com ausência do CAP unilateral. É descrita a técnica de reconstrução da papila com retalho local em C-V associado à tatuagem para pigmentação do CAP em tempo único. Foram apresentados os dados referentes ao tipo de reconstrução mamária realizada, às complicações e à presença de radioterapia prévia. Resultados: A maioria das pacientes foi reconstruída com implante (48%). Houve três complicações relacionadas à papila (14%), uma necrose parcial e duas deiscências pequenas. Não houve complicação relacionada à tatuagem. Onze pacientes (52%) haviam realizado radioterapia prévia na mama. Conclusão: A tatuagem associada ao retalho em C-V em tempo único é uma técnica simples e com baixo índice de complicações.


Introduction: Reconstruction of the nipple-areola complex (NAC) is a fundamental step in the treatment of breast cancer. It is usually performed in two distinct stages. Tattooing is performed only after reconstruction and complete scarring of the nipple. Methods: This retrospective study was conducted from 2015 to 2016 at the Hospital São Lucas of the Pontifical Catholic University of Rio Grande do Sul. Twenty-one patients with a single NAC were included. One-stage reconstruction of the nipple using a local C-V flap associated with tattooing for pigmentation of the NAC is described. Data on the type of breast reconstruction performed, associated complications, and presence of previous breast radiotherapy were presented. Results: Most patients (48%) underwent reconstruction with an implant. Three complications (14%) were observed in the nipple, including one case of partial necrosis and two cases of small dehiscence. None of the patients had complications due to tattooing. Eleven patients (52%) underwent breast radiotherapy previously. Conclusion: Tattooing combined with one-stage reconstruction using a C-V flap is a simple technique with a low rate of complication.

INTRODUÇÃO: Pacientes com gigantomastia apresentam múltiplos sintomas físicos e psicossociais. Várias técnicas foram propostas para o seu tratamento. A amputação mamária, descrita por Torek em 1922, apresenta com excelente alternativa, porém com prejuízo na funcionalidade mamilar e no formato da mama. Liacyr Ribeiro, em 1975, descreveu o retalho inferior dermoglandular não areolado a fim de proporcionar tecido de segurança para ressecção mamária, facilitar a montagem da mama e melhorar sua projeção. Este trabalho propôs unir estas duas consagradas técnicas, visando aprimorar os resultados das amputações mamárias. MÉTODOS: Foram operadas 11 pacientes com gigantomastia com prole definida, pela técnica de amputação mamária associada ao pedículo inferior não areolado. RESULTADOS: Distância média da fúrcula esternal ao mamilo foi de 35,6 centímetros na mama direita e 35 centímetros na mama esquerda, variando de 30 a 44 centímetros. A ascensão do complexo areolomamilar foi em média de 16,9 centímetros na mama direita e 16,4 centímetros na mama esquerda, variando de 12 a 25 centímetros. A quantidade de ressecção média de tecido mamário por paciente foi de 3559,5 gramas, variando de 1600 a 5890 gramas. A hipopigmentação do complexo areolamamilar esteve presente em três (27%) pacientes. A deiscência do T foi observada em dois (18%) pacientes. A não integração parcial do enxerto ocorreu em três (27%) das pacientes, com perdas estimadas de 10%, 30% e 80% do enxerto. CONCLUSÃO: A associação da amputação mamária com o pedículo dermoglandular inferior não areolado nos oferece um melhor remodelamento da mama, segurança na montagem desta, além de proporcionar uma adequada projeção da mesma.


INTRODUCTION: Patients with gigantomastia have multiple physical and psychosocial symptoms. Various techniques have been proposed for their treatment. Described by Torek in 1922, mammary amputation was presented as a great alternative, but resulted in reduced mammillary functionality and loss of breast format. In 1975, Liacyr Ribeiro described the use of dermaglandular inferior pedicle as safety tissue to allow for mammary resection, to facilitate breast assembly and to improve projection. The author proposed to unite these two consolidated techniques with the intention of improving breast amputation outcomes. METHODS: Eleven gigantomastia patients were operated on by means of the amputation technique using dermaglandular inferior pedicle. RESULTS: The mean distance between the sternal notch and the nipple was 35.6 cm for the right breast and 35 cm for the left breast, with all measures ranging between 30 cm and 44 cm. Rise of the nipple-areola complex was in average 16.9 cm for the right breast and 16.4 cm for the left breast, varying from 12 to 25 cm. The amount of breast tissue resection per patient was, in average, 3559.5 grams, ranging from 1600 grams to 5890 grams. Hypopigmentation of the nipple-areola complex was present in three patients (27%). Dehiscence of the T was observed in two patients (18%). Partial non-integration of the graft occurred in three patients (27%), with loses estimated at 10%, 30% and 80% of the graft. CONCLUSION: Associating mammary amputation with an inferior dermaglandular pedicle provides good remodeling and safe assembling of the breast, in addition to providing proper projection.

INTRODUÇÃO: A hipertrofia mamária é uma alteração do contorno corporal bastante frequente, e de grande importância clínica, já que repercute na mulher tanto psicologicamente como funcionalmente, em especial nas grandes hipertrofias. Para evitar o sofrimento vascular e necroses nos casos de mamoplastias redutoras em grandes hipertrofias e ptoses severas, uma opção segura é a realização de mamoplastia redutora a Pitanguy, associada à confecção de retalho dermoglandular areolado superomedial de Silveira Neto. MÉTODOS: Foram realizadas, no período de abril de 2014 a julho de 2016, 14 mamoplastias redutoras utilizando a técnica descrita. A ressecção de parênquima variou entre 900 gramas a 1.800 gramas, e a distância de deslocamento superior do complexo areolopapilar variou entre 5 e 15 centímetros. Observou-se a incidência de complicações vasculares e cicatriciais do complexo areolopapilar. RESULTADOS: Todas as pacientes referiram satisfação com o resultado estético e funcional obtidos pela cirurgia. Em quatro pacientes, houve pequena área de epidermólise na região de encontro do sulco submamário com a cicatriz vertical, que cicatrizaram por segunda intenção, sem causar grandes sequelas estéticas. Em nenhum dos casos houve epidermólise ou necrose parcial ou total do complexo areolopapilar, apesar de grandes ascensões dos mesmos. Houve dois casos de hipocromia em pequena área da margem areolar. CONCLUSÃO: Para o tratamento de grandes hipertrofias mamárias, a combinação das técnicas selecionadas foi bem-sucedida em possibilitar grandes reduções volumétricas, correção de ptoses severas, com elevação do complexo areolopapilar por distâncias significativas, com segurança. Desta forma, foram obtidos bons resultados estéticos e funcionais, sem sofrimentos vasculares do complexo areolopapilar.


INTRODUCTION: Mammary hypertrophy is common, and has great clinical importance, affecting women both psychologically and functionally, especially when severe. To avoid vascular compromise and necrosis associated with reduction mammoplasty in cases with severe hypertrophy and ptosis, a safe option involves combined use of Pitanguy technique and a Silveira Neto areolar superomedial dermoglandular flap. METHODS: Fourteen reduction mammoplasties were performed between April 2014 and July 2016 with the combined technique. Parenchymal resection ranged from 900 to 1,800 g, and the superior displacement distance of the nipple-areolar complex ranged from 5 to 15 cm. Vascular and scarring complications of the nipple-areolar complex were evaluated. RESULTS: All patients reported satisfaction with the aesthetic and functional results obtained with surgery. A small area of sloughing where the inframammary crease and the vertical scar intersect was observed in 4 patients; healing occurred by second intention, without significant aesthetic sequelae. Neither sloughing nor partial or total necrosis of the nipple-areolar complex were observed, despite the significant elevation. Two cases of hypopigmentation of a small area at the areolar margin were observed. CONCLUSION: The combined technique was successful in the treatment of prominent mammary hypertrophy, enabling safe reduction of large volumes, correction of severe ptosis, and significant elevation of the nipple-areolar complex. Good aesthetic and functional results were obtained, without vascular compromise of the nipple-areolar complex.

Introducción: La ginecomastia define el aumento benigno del tamaño de la glándula mamaria en el hombre. Existen diversos abordajes quirúrgicos para la resección de la lesión, cada una con resultados diferentes. Material y métodos; Estudio transversal. Se incluyeron pacientes con ginecomastia operados de mastectomía subdérmica mediante incisión periareolar externa e incisión periareolar inferior. Para la aleatorización se tomó en cuenta los grados IIb y III de Simon, distribuyendo uno a uno para cada tipo de incisión. Los resultados estéticos fueron evaluados por un cirujano experimentado, tomando como excelentes cuando hubo una cicatrización correcta sin deformidad del área, buenos cuando la cicatrización fue buena sin deformidad del área y mala cuando hubo deformidad del área operada. Resultados: Fueron operados 24 pacientes, el 50% por incisión periareolar externa y el 50% por incisión periareolar inferior. La edad promedio fue 25,58 y 27,58 años respectivamente, sin diferencias significativas p = 0,513. Todos los pacientes tuvieron características sexuales secundarias normales. La etiología fue idiopática en 23 pacientes (95,83%). El tiempo promedio de evolución fue 32,28 meses y en todos el resultado histopatológico fue ginecomastia. La evaluación del aspecto estético de la incisión y el área afectada en los pacientes operados mediante incisión periareolar externa (n = 12) fue mala en un paciente (8,33%), buena en 2 (16,66%) y excelente en 9 pacientes (75%), mientras que en los pacientes operados mediante incisión periareolar inferior, fue buena en el 100% de los pacientes, y mala y excelente en ningún paciente, con diferencias estadísticamente significativas para ambas incisiones, p = 0,000. Ningún paciente presentó complicaciones. Conclusión: Ambas incisiones son seguras. La incisión periareolar externa ofrece mejores resultados que la incisión periareolar inferior para realizar mastectomía subdérmica en pacientes con ginecomastia en todos los grados Simon.


Introduction: Gynecomastia defines the benign enlargement of the mammary gland in man. There are several surgical approaches for resection of the lesion, each with different results. Material and methods: Cross-sectional study. There were included patients with gynecomastia, operated by means of external and inferior periareolar incision. For randomization was took into account degrees IIb and III of Simon, distributing one to one for each type of incision, the cosmetic results were evaluated by an experienced surgeon, taking as excellent results when there was a proper healing without deformity of the area, good results when healing was good without deformity, and bad results when there was deformity of the operated area. Results: There were 24 patients, 50% operated by external periareolar incision and 50% by lower periareolar incision. The mean age was 25.58 and 27.58 years old for each group, with no significant statistically differences (P = .513). All patients had normal secondary sexual characteristics. The etiology was idiopathic in 23 (95.83%). The average evolution time was 32.28 months, on all histopathological result was gynecomastia. The evaluation of the aesthetic aspect of the incision and the area affected in patients operated by external periareolar incision (n = 12) was bad to 1 (8.33%), good in 2 (16.66%) and excellent in 9 (75%) patients; 100% of the patients operated by inferior periareolar incision presented good results, there were statistically significant differences for both incisions, P = .000. There were no complications. Conclusion: Both incisions are safe, periareolar external incision provides better results than the inferior periareolar incision for patients with gynecomastia in all degrees of Simon.

Introdução: A reconstrução do complexo areolomamilar é a fase final da reconstrução mamária. As técnicas de reconstrução do mamilo mais utilizadas são com retalhos locais ou enxertos. A utilização destas técnicas em mamas que sofreram queimaduras apresenta resultados indesejados, em decorrência da menor vascularização da pele lesada. O objetivo deste trabalho foi avaliar a utilização do retalho trilobado autonomizado na reconstrução do mamilo em mamas queimadas. Métodos: Oito pacientes do sexo feminino tiveram seus mamilos reconstruídas em dois tempos cirúrgicos. Resultados: Não ocorreram complicações, como necrose, deiscência, perda completa da projeção do novo mamilo ou infecção. A projeção média ao término da cirurgia foi de 15,25 mm e, após seis meses de reconstrução, foi de 3 mm, permanecendo estável. Todas as pacientes ficaram satisfeitas com os resultados. Conclusão: Concluímos que a autonomização deu segurança à reconstrução do mamilo em mamas queimadas e manteve projeção mínima satisfatória do mamilo reconstruído.


Introduction: Reconstruction of the nipple-areola complex is the final stage of breast reconstruction. The most common nipple reconstruction techniques use local flaps or grafts. However, these techniques in cases of burns produce undesirable outcomes due to the decreased vascularization of damaged skin. The objective of this work was to evaluate the use of the autonomized star flap in the nipple reconstruction of burned breasts. Methods: Nipples were reconstructed in eight female patients in two surgeries each. Results: There were no complications such as necrosis, dehiscence, complete loss of projection of the new nipple, or infection. The mean projection at the end of surgery was 15.25 mm; 6 months after reconstruction, it was 3 mm, showing stability. All patients were satisfied with their results. Conclusion: We conclude that autonomization leads to safe reconstruction of the nipple on burned breasts and maintains a satisfactory minimal projection of the reconstructed nipple.

INTRODUÇÃO: A cirurgia de mastopexia associada ao aumento mamário vem sendo cada vez mais solicitada. Ao mesmo tempo, pacientes com graus menores de ptose aceitam menos as correções por meio de cicatrizes verticais. Nesse contexto, a mastopexia periareolar (circum-areolar) com implante cônico é uma opção que resulta em cicatriz limitada ao perímetro areolar e possibilita o tratamento das alterações de posição e tamanho do complexo aréolo-papilar. MÉTODO: Foram avaliadas 22 pacientes, submetidas à mastopexia periareolar com utilização de próteses cônicas, revestidas com poliuretano, colocadas em posição subglandular e operadas pelo mesmo cirurgião. RESULTADOS: Dados observados: 45% das pacientes apresentavam ptose grau I; 32%, grau II, e 23%, grau III; 86% das pacientes apresentavam assimetria do complexo aréolo-papilar; 27%, lateralização, e 18%, aréolas grandes, havendo associações na mesma paciente. O volume das próteses variou de 215 mL a 380 ml. Houve dois casos de alargamento da cicatriz, mas não houve casos de alargamento de aréola ou cicatriz hipertrófica. Não houve, no período pesquisado, complicações relacionadas à colocação do implante. No questionário de satisfação, a maioria das pacientes considerou o aspecto da mama natural, conferindo boas notas aos aspectos: formato, simetria e altura das mamas, qualidade da cicatriz e posição, formato e tamanho das aréolas. CONCLUSÕES: A técnica permite resultados satisfatórios nos casos indicados, como ptoses leves a moderadas, aréolas grandes, lateralização e assimetria do complexo aréolo-papilar. São aspectos técnicos importantes: a retirada conservadora de pele periareolar, o uso de implantes de tamanhos moderados e a confecção da sutura de contenção em round-block.


INTRODUCTION: Mastopexy surgery associated with breast augmentation is increasingly being requested. At the same time, patients with lower degrees of ptosis are less receptive to any correction through vertical scars. In this context, periareolar (circumareolar) mastopexy with a conical implant is an option that results in a scar that is limited to the areolar perimeter, and enables the treatment of changes in the position and size of the nipple-areola complex. METHOD: We evaluated 22 patients submitted to periareolar mastopexy with use of conical prostheses coated with polyurethane and placed in a subglandular position. All patients were operated on by the same surgeon. RESULTS: Among the patients, 45% presented with grade I ptosis, 32% grade II, and 23% grade III, and 86% exhibited asymmetry of the nipple-areola complex, 27% lateralization, and 18% large areolas, with some combination in the same patient. The volume of the prostheses ranged from 215 to 380 mL. There were two cases of scar enlargement; however, there were no cases of enlargement of the areola or hypertrophic scar. During the period studied, there were no complications related to the placement of the implant. In the satisfaction questionnaire, most of the patients considered the aspect of the breast to be natural in appearance, giving good grades to the size, symmetry, and height of the breasts; the quality of the scar; and the position, shape, and size of the areolas. CONCLUSIONS: This technique produces satisfactory results in cases of mild to moderate ptoses, large areolas, lateralization, and asymmetry of the nipple-areola complex. Important technical aspects include the conservative removal of periareolar skin, use of implants of moderate sizes, and the use of the round-block containment suture.

INTRODUÇÃO: A confecção do complexo areolopapilar representa o estágio final nas reconstruções de mama, e talvez seja o mais difícil deles. Diversas técnicas são descritas, entretanto, a maioria delas não parece atingir a projeção adequada e sofre com a perda dessas com o passar do tempo, levando a um resultado final estético inadequado. O objetivo do trabalho é comparar e avaliar os resultados e eficácia de quatro técnicas cirúrgicas diferentes de confecção de papila, em cirurgias de reconstrução de mama nos últimos 3 anos. MÉTODO: Foram avaliados 64 papilas reconstruídas, utilizando as técnicas de skate flap (n=17), double opposing flap (n=15), "4 pétalas" (n=22) e enxerto de papila contralateral (n=10). Os resultados foram avaliados, a longo prazo, por um examinador cego que classificou os resultados como totalmente satisfatório, satisfatório, parcialmente satisfatório e insatisfatório. RESULTADOS: O enxerto de papila apresentou resultados significativamente melhores que dos outros grupos (p= 0,012). Não houve diferença estatística quando, somente, avaliados os retalhos (excluindo o enxerto de papila)(p=0,102). Já avaliando o resultado da papila confeccionada em função do tipo de reconstrução primária feita, não houve diferença entre os grupos (p=0,563). CONCLUSÃO: O enxerto de papila contralateral se mostrou uma técnica mais efetiva e com melhores resultados. Na impossibilidade do seu uso, não há diferença entre as outras técnicas avaliadas, ficando a sua indicação àquela que se sinta mais familiarizado e capacitado a realizar.


NTRODUCTION: Reconstruction of the nipple-areola complex represents the final and perhaps most difficult stage in breast reconstruction. Several techniques have been described; however, most of the methods do not seem to achieve adequate projection and are associated with the loss of projection over time, leading to an inadequate final aesthetic result. The objective of this study is to compare and evaluate the results and effectiveness of four different surgical nipple reconstruction techniques in the last 3 years. METHOD: Sixty-four reconstructed nipples were evaluated using the skate flap (n = 17), double-opposing-tab flap (n = 15), four petals (n = 22), and contralateral nipple graft (n = 10) techniques. The results were evaluated in the long term by a blinded examiner who classified the results as fully satisfactory, satisfactory, partially satisfactory, or unsatisfactory. RESULTS: The nipple graft showed significantly better results than the other groups (p = 0.012). There was no statistical difference when only the flaps (excluding the contralateral nipple graft) were evaluated (p = 0.102). However, when evaluating the outcome of nipple reconstruction according to primary reconstruction type, no difference was observed among the groups (p = 0.563). CONCLUSION: The contralateral nipple graft technique proved to be the most efficient method that yielded better results; however, no difference was seen among the other evaluated techniques. Hence, when the contralateral nipple graft technique is not possible, surgeons should select the one with which they are most familiar.

INTRODUÇÃO: A reconstrução do complexo areolopapilar (CAP) é etapa fundamental na reconstituição mamária nos casos em que há amputação desse complexo durante a mastectomia. Uma técnica muito eficiente é a do double opposing flap, que possibilita reconstruir o CAP, propiciando diâmetro adequado, boa projeção e simetria em relação ao CAP contralateral, com a possibilidade de fechar a área doadora e com todas as cicatrizes contidas na topografia da nova aréola reconstruída. O objetivo deste estudo é demonstrar os resultados obtidos nas reconstruções do CAP com o double opposing flap nas reconstituições mamárias. MÉTODO: Estudo retrospectivo de 24 pacientes, nas quais foram reconstruídos 31 CAPs (17 unilaterais e 7 bilaterais) utilizando a técnica referida, entre julho de 2008 e junho de 2010. Os resultados foram avaliados objetiva e subjetivamente. RESULTADOS: Na análise subjetiva, o grau de satisfação das pacientes foi elevado no que concerne ao resultado cirúrgico final. Na análise objetiva, o diâmetro horizontal areolar e a projeção mamilar dos CAPs reconstruídos demonstraram-se matematicamente semelhantes, com valores de P apresentando significância estatística (P > 0,05). CONCLUSÕES: O double opposing flap é, na atualidade, uma excelente estratégia para reconstrução do CAP, com metodização que propicia curta curva de aprendizado, garantindo mamilos centralizados, simétricos e com resultados duradouros.


BACKGROUND: Reconstruction of the nipple-areola complex (NAC) is the fundamental step in breast reconstruction in cases involving amputation of the nipple complex during mastectomy. The double opposing flap technique enables efficient reconstruction of the NAC, providing an adequate diameter, good projection, and symmetry with respect to the contralateral NAC. In addition, the donor area can be closed, and all scars can be contained within the topography of the reconstructed areola. This study presents the results obtained in NAC reconstruction using the double opposing flap technique in breast reconstruction. METHODS:This retrospective study involved 24 patients in whom 31 NACs (17 unilateral and 7 bilateral) were reconstructed using the abovementioned technique between July 2008 and June 2010. The results were evaluated objectively and subjectively. RESULTS: In the subjective analysis, the patients' satisfaction level regarding the final surgery results was high. In the objective analysis, the areolar horizontal diameter and nipple projection of the reconstructed NACs were quantitatively similar (P > 0.05). CONCLUSIONS: The double opposing flap is an excellent strategy for NAC reconstruction. The method has a short learning curve and guarantees centralized symmetrical nipples with long-term results.

ABSTRACT

A doença de Mondor é uma rara afecção, caracterizada pela tromboflebite dos vasos superficiais da região toracoabdominal, ocorrendo como uma complicação da cirurgia de mama. O objetivo deste estudo é, a partir de um caso clínico, fazer uma revisão da literatura científica sobre a doença de Mondor. O presente artigo é o relato de caso de um paciente do sexo masculino, de 38 anos de idade, portador de hipertrofia das glândulas mamárias e do mamilo direito, com prejuízos estéticos e psicológicos. Após avaliação inicial, o paciente foi submetido à ressecção parcial do mamilo direito e das glândulas mamárias por via periareolar inferior. No período pós-operatório, o paciente desenvolveu cordões dolorosos e eritematosos na região torácica inframamária bilateralmente, na topografia das veias toracoepigástricas. Os achados eram compatíveis com doença de Mondor. O paciente foi tratado com gel à base de heparina e analgésicos por via oral. Após tratamento, o paciente permaneceu com uma linha endurecida à direita. A doença de Mondor representa rara e benigna complicação da cirurgia da mama, que deve ser tratada de forma sintomática. É importante que o cirurgião esteja apto para o diagnóstico e para orientar o paciente sobre o caráter autolimitado da doença de Mondor.


Mondor's disease is a rare condition characterized by thrombophlebitis of the superficial vessels of the thoracoabdominal region that occurs as a complication of breast surgery. The aim of this study is to review the scientific literature on Mondor's disease in light of the currently reported clinical case. In this article, we describe the case of a 38-year-old man with hypertrophy of the mammary glands, which produced aesthetic and psychological effects. After an initial evaluation, the patient underwent partial resection of the right nipple and the mammary glands by inferior periareolar approach. In the postoperative period, the patient developed painful, erythematous cords in the bilateral inframammary thoracic region following the topography of the thoracoepigastric veins. The findings were consistent with Mondor's disease. The patient was treated with gel-based heparin and oral analgesics. After treatment, a hard line remained on the right side. Mondor's disease is a rare and benign complication of breast surgery that should be treated symptomatically. It is important that surgeons are able to diagnose and counsel patients about the self-limiting characteristics of Mondor's disease.

INTRODUÇÃO: A reconstrução do complexo areolopapilar (CAP) constitui o passo final na criação da neomama, com o objetivo de aproximá-la da mama contralateral após a mastectomia. A restauração do CAP tem sido historicamente um procedimento feito em estágios, com a reconstrução da papila realizada por meio de enxertos ou retalhos locais, previamente, e da aréola, posteriormente, buscando a coloração adequada. Atualmente, a utilização da tatuagem areolar está alcançando popularidade cada vez maior, comparativamente às técnicas tradicionais. O objetivo deste trabalho é descrever a técnica e os equipamentos utilizados para a tatuagem intradérmica em pacientes mastectomizadas, realizada no Serviço de Cirurgia Plástica e Microcirurgia Reconstrutiva do Hospital Universitário Walter Cantídio (Fortaleza, CE, Brasil). MÉTODO: O presente trabalho descreve os passos para a realização do procedimento reparador utilizando a tatuagem areolar permanente com equipamento convencional de tatuagem artística profissional. O procedimento foi aplicado em 10 pacientes submetidas a reconstrução mamária pós-mastectomia. RESULTADOS: O método demonstrou as vantagens da simplicidade técnica do procedimento, não requerendo internamento nem resultando em morbidade de áreas doadoras. CONCLUSÕES: A tatuagem do CAP é um procedimento seguro, rápido, com baixa morbidade e bons resultados na finalização da reconstrução mamária.


BACKGROUND: The final step in the creation of the neo-breast is reconstruction of the nipple-areolar complex (NAC), which is aimed toward bringing the appearance of the NAC closer to that of the contralateral breast following mastectomy. NAC restoration has historically been a stepwise procedure, in which reconstruction of the nipple by the use of grafts or local flaps was followed by reconstruction of the areola to achieve the correct color. Currently, the popularity of the areolar tattoo is increasing compared to traditional techniques. The aim of this study was to describe the technique and equipment used for intra-dermal tattooing in mastectomized patients at the Plastic Surgery and Reconstructive Microsurgery Service of the Walter Cantídio University Hospital (Fortaleza, CE, Brazil). METHODS: This study describes the steps of the repair procedure by using permanent areolar tattooing with conventional equipment from a professional tattoo artist. The procedure was used in 10 patients undergoing post-mastectomy breast reconstruction. RESULTS: The advantages of this method included the technical simplicity of the procedure, the ability to use this procedure in outpatients, and the lack of morbidity of the donor areas. CONCLUSIONS: Tattooing the NAC is a safe, fast procedure with low morbidity and good results of breast reconstruction.

A adenomatose erosiva do mamilo é uma complexa proliferação benigna mamária que pode ser confundida com neoplasias malignas da mama. A apresentação típica cursa com descarga mamária, eritema, erosão e formação de crostas. O processo é geralmente assintomático e de instalação insidiosa. A adenomatose erosiva do mamilo pode ser confundida com condições benignas, como a dermatite de contato, psoríase e infecções, mas seu principal diagnóstico diferencial é a Doença de Paget. O tratamento é cirúrgico e o prognóstico, excelente.


Erosive adenomatosis of the nipple is a complex benign mammary proliferation that can be misdiagnosed as a malignant mammary neoplasm. The most common clinical presentation includes discharge, erythema, erosion and crusting. The process is usually asymptomatic. It resembles benign conditions such as contact dermatitis, psoriasis and infections, but its main differential diagnosis is Paget's disease. Treatment is usually surgical and the prognosis is excellent.

OBJECTIVES: This paper discusses the influence of a para-areolar incision in the upper outer quadrant of the breast on the location of the sentinel lymph node in a canine model. METHODS: The sentinel lymph node was marked with technetium-99, which was injected into the subareolar skin of the cranial breast. After the marker had migrated to the axilla, an arcuate para-areolar incision was performed 2 cm from the nipple in the upper outer quadrant. Patent blue dye was then injected above the upper border of the incision. At the marked site, an axillary incision was made, and the sentinel lymph node was identified by gamma probe and/or by direct visualization of the dye. The agreement between the two injection sites and the two sentinel lymph node identification methods was determined. Our sample group consisted of 40 cranial breasts of 23 adult females of the species Canis familiaris. The data were analyzed by using the McNemar test and by determining the kappa agreement coefficient. RESULT: Our findings showed that in 95 percent of the breasts, the sentinel lymph node was identified by the injection of technetium-99 m into the subareolar region, and in 82 percent of the cases, the sentinel lymph node was identified by the injection of patent blue dye above the upper border of the incision. The methods agreed 82 percent of the time. CONCLUSIONS: Previous para-areolar incisions in the upper outer quadrant did not interfere significantly with the biopsy when the dye was injected above the upper border of the incision.

ABSTRACT

Reportar y describir las características clínicas y conducta terapéutica de 13 casos de enfermedad de Paget en el Instituto de Oncología “Dr. Miguel Pérez Carreño” en el período comprendido entre 1986-2008. Se realizó un estudio no experimental, descriptivo, de tipo retrospectivo. Se revisaron historias clínicas del departamento de dicha institución. Total de casos encontrados: 13, 100 por ciento de los pacientes fueron del sexo femenino. La edad promedio de presentación fue de 58 años. 44 por ciento de los casos se presentó con una lesión eccematosa a nivel de pezón y un 14 por ciento se encontraban en estadio I en el momento de diagnóstico. 87 por ciento presentaban extensión centro-mamaria; el tiempo de evolución fue menor o igual a 6 meses en un 50 por ciento de los casos. El síntoma de presentación en un 45 por ciento fue la presencia de un nódulo mamario más lesión ulcerativa del pezón. 25 por ciento cursaba con adenopatía clínicamente palpable. En un 65 por ciento, la conducta quirúrgica fue mastectomía total con vaciamiento axilar en niveles I y II. 37 por ciento recibieron tratamiento adyuvante y en un 72 por ciento el tiempo de sobrevida comprendió entre el rango de 1 a 5 años. La enfermedad de Paget constituye una entidad especial poco frecuente. El grupo etario afectado está entre 50-60 años. El tratamiento terapéutico establecido en la mayor parte de las series publicadas, independientemente de la etapa, ha sido la mastectomía radical.


The objective of this study was report and describes the clinical characteristics and therapeutically conducts in 13 cases of Paget disease view in the Oncology Institute Dr. Miguel Perez Carreño between the years 1986-2008. We realized a study not experimental, descriptive and retrospective. We review the clinical department history of our institution. The total cases found was 13, 100 percent of the patients were female. The average age at the presentation was 58 years old. The 44 percent of the cases were presented with an eczematous lesion in the nipple level, 14 percent of the cases were in stage I at the diagnostic moment. The 87 percent of cases present a center mammalian extension to the diagnostic moment. The evolution time were minor or equal to 6 month in a 50 percent of the cases. The dominant symptom in 45 percent was the presence of a mammalian nodule and an ulcerative lesion in the nipple. In the 66 percent of the cases the surgical conduct was a total mastectomy with axillaries dissection in the level I and II. Only 37 percent receive adjuvant treatment and in a 72 percent the super life were between the 1 and 5 years range. Paget disease is a less special entity. The average groups affected are between the 50 and 60 years old. The established therapeutically treatment in the majority of series published independent of the stage was the surgical.

ABSTRACT

Introdução: A reconstrução mamária está completa quando é realizada a reconstrução do complexo aréolo-papilar (CAP). Modelos de treinamento têm agregado benefícios, sendo exercícios de simulação. Apresenta-se um modelo para treinamento em reconstrução do CAP. Método: Selecionou-se dez pacientes, nos quais a zona IV do retalho miocutâneo transverso do músculo reto abdominal (TRAM) foi utilizada para instrução em duas técnicasde retalhos para confecção do CAP. Resultados: O modelo de treinamento foi aplicado em todos os casos, tendo os residentes adquirido habilidades e domínio da técnica, demonstrando segurança na reconstrução dos casos clínicos. Conclusão: A reconstrução do CAP com zona IV do retalho TRAM é um modelo de treinamento útil.


Background: Breast reconstruction is complete when the nipple-areola complex (NAC)is reconstructed. Models can be used to train young surgeons, it been a simulator exercise. A model of NAC reconstruction training is the purpose of the this paper. Methods: Ten zone IV TRAM flaps were utilized to the purpose of the study. First, the trainee executes the NAC reconstruction using the flaps techniques supervised by a teacher. Afterwards, the technique skills are used on real situation in the operation room. Results: The model was used by all trainees who acquire technique skills, which was used on real situation. Conclusion: The NAC reconstruction in zone IV TRAM flap is feasible.

ABSTRACT

Introdução: Os resultados quanto à manutenção da projeção e tamanho em longo prazo da papila quando da reconstrução do complexo aréolo-papilar são insatisfatórios em até 70% dos pacientes. Por meio de revisão de literatura, há consenso da necessidade de um preenchimento associado a retalhos cutâneos na formação e durabilidade de forma e projeção da neopapila, prejudicados pelas forças de contração dos tecidos locais e do retalho em si. Referidos preenchimentos podem ser autólogos (gordura, dermo-gorduroso, cartilagem costal, cartilagem auricular) ou heterólogos (politetrafluoroetileno, hidroxiapatita de cálcio, alloderm®, goretex®, ácido hialurônico). Conclusão: Concluímos que, atualmente, o preenchimento autólogo com cartilagem auricular ou dermo-gorduroso parecem ser aqueles de menor morbidade cirúrgica, menor custo financeiro e melhor resultado tardio.


Introduction: Many techniques have been used by plastic surgeons to achieve nipple reconstruction. Although numerous nipple reconstruction techniques are available, all have been plagued by eventual loss of long-term projection. Even using several cutaneous flaps, there are lose of nipple’s projection influenced essentially by two factors, i.e., tissue contraction of the flap and retraction forces of surrounding (underlying) tissues. The use of supplemental material to increase the volume of the nipple may be autologous homografts (fat, dermis, auricular cartilage, costal (rib) cartilage) or synthetic materials (a cellular dermamatrix, gore-tex®, polytetrafluoroethylene, hyaluronic acid, calcium hydroxylapatite). Conclusion: We have concluded that combination of a cartilage graft or dermis graft and a cutaneous flap reestablishes the soft tissue and rigidity required for appropriate nipple reconstruction with minimal surgical morbidity and low coasts with long-term nipple projection.
